Licensing

LicensE is DSPS's new, online, self-guided occupational license application platform.

  • Click on the "Register for an Individual" button. Complete this log in user information and password form.
  • Once you have a username and password, log into the LicensE portal.
  • Click on "Apply for a License"
  • In the "Category" drop down box, select "Health"
  • In the "License Types" drop down box, select "Marriage and Family Therapist" or "Marriage and Family Therapist Training License"
  • You will see several choices for you to select from:
    • Act 10: This license can be issued to a provider from another state during a national public health emergency. This license can also be issued to providers who are licensed in good standing in another state who also apply for a permanent license at the same time.
    • Initial Application: Initial application for Marriage and Family Therapist. This is NOT the training license. If you need to apply for the training license to gain supervised experience, press Previous and select Marriage and Family Training License from the professions list. See form 12401 for more information: https://dsps.wi.gov/Credentialing/Health/fm12401.pdf
    • Licensure by Reciprocity: Application by Reciprocity may be an option for a professional who holds a license in another state or jurisdiction. An application by Reciprocity is solely based on the jurisdictional standards of the state you are coming from compared to the WI requirements. This determination does not include your individual qualifications. See form 12403 for more information: https://dsps.wi.gov/Credentialing/Health/fm12403.pdf
    • Renewal After 5 Years: For applicants reapplying after five years have passed since previously held license expired. https://dsps.wi.gov/Credentialing/Health/fm12406.pdf
    • Temporary License: An applicant for full licensure may also apply for a temporary certificate at the same time. Eligible applicants must also submit or have previously submitted an application for full licensure. Note: a temporary license is only available for those applying for Initial Application. See form 12502 for more information: https://dsps.wi.gov/Credentialing/Health/fm12502.pdf
